Legal Documents required to be attached to the Polk HCAP System
The following documents have been approved by the legal department of the Polk County Board of County Commissioners, please download them and review them with your respective agencies. The Intra Agency agreement must be signed and passed through the BoCC before you connect to the live system.

| HCAP System Client Authorization Release | Must be signed by client and attached to medical record |
| HCAP System Confidentiality Agreement | Signed by trainees before they get their login to the system |
| HCAP Intra Agency Agreement Sample | Original must be provided by the system admin with typed name of agencies contact the system admin for you copy. You can use this document for pre approval in your legal department however an original must be provided by the system admin. |
