Polk County’s health care community is coming together to ensure access to high quality care for people living with chronic diseases. The Quality Management Committee is aligning forces to drive quality improvements through adopting uniform quality standards of care, measuring performance as a community, educating the public on quality health care for improving chronic conditions, and working with providers and the community to remove barriers to quality care. The Committee's goal is to increase the number of people who successfully manage their chronic diseases in partnership with their provider, through accessible quality care. Health care system performance on key quality indicators will be shared on this site in the future.
